Cos'è
Pancreatic cancer and biliary tract cancers have both an increasing incidence and are often diagnosed at an advanced stage, not amenable to curative treatment. Moreover, they share some clinical features such a first clinical presentation commonly associated with non-specific symptoms, and the possible appearance of jaundice, with related need for drainage. Finally, their management is strictly dependent on a multidisciplinary evaluation. On the other hand, they have different therapeutic paths. In pancreatic cancer, neoadjuvant therapy for resectable disease is currently an established approach. In the advanced setting, new treatment opportunities have arrived, but more are needed, and a significant contribution is awaited from a more comprehensive molecular evaluation. In biliary tract cancer, neoadjuvant treatment for resectable disease is a new therapeutic horizon, and trials are ongoing to explore this route. In advanced disease, the advent of immunotherapy and several targeted therapies lead to unprecedented responses and long survival. As a consequence of brilliant results from new systemic therapies and of their integration with increasingly available locoregional therapies such as TARE, new scenarios are disclosed for patients, and even liver transplantation is a possible new option for selected cases. Given these premises, in this event we aim to depict state-of-the-art therapeutic approach for pancreatic and biliary tract cancers, and try to unveil the possible future directions, always keeping focus on the necessary multidisciplinary management in order to achieve the best results for patients.
